Qualifications

• Experience with EHRs and basic data tracking and reporting.

Responsibilities

• Coordinate daily discharge planning support activities under leadership supervision.
• Prioritize assignments, track workload, and ensure timely patient transitions.
• Coordinate referrals, notices, and post-acute service arrangements with teammates.
• Provide orientation and guidance to staff on discharge processes and resources.
• Maintain patient records, protect confidentiality, and support payer documentation.
• Collaborate with case managers, clinicians, payers, and community providers.
• Participate in rounds and meetings, identify barriers, and escalate as needed.
• Collect and review discharge data and department metrics for reporting.
