Job summary

Qualifications

• RN licensure in Georgia or multi-state licensure required.
• LMSW licensure or eligibility required.
• Master’s degree in Social Work required.
• Associate’s Degree in Nursing required; Bachelor’s preferred.
• Minimum 2 years in care management or related field.

Responsibilities

• Coordinate and monitor all Care Management team activities.
• Provide leadership, coaching, and mentoring to Care Management staff.
• Lead Discharge Planning and Transitions of Care in the hospital setting.
• Monitor quality indicators to ensure appropriate social and transitional services.
• Develop relationships with physicians, nursing supervisors, payers, and community resources.
