Job summary

Qualifications

• Associate Degree in Nursing and RN license in Vermont in good standing.
• Minimum of three years of clinical discharge planning, utilization and quality management experience.
• Ability to review charts, extract data and analyze care processes.

Responsibilities

• Lead the discharge planning process beginning at admission or prior to planned hospitalization.
• Coordinate interdisciplinary care with patients, families, and providers.
• Utilize evidenced-based discharge planning elements and teach-back methods.
