• Ability to coach, develop, and lead a multidisciplinary team.

Responsabilidades

• Oversee discharge-planning care operations and ensure effective, patient-centered case management.
• Collaborate with clinicians and external providers to promote safe transitions and continuity of care.
• Lead staff development, including coaching and performance support for the case management team.
• Ensure compliance with applicable standards, policies, and regulatory requirements.
• Drive performance improvement initiatives and monitor key outcomes.
• Manage resource allocation and departmental reporting.
• Oversee departmental budgets and support fiscal stewardship.
