Job summary

Saint Mary's Regional Health System is seeking a dedicated Discharge Planner to coordinate patient transitions from hospital to home or other care settings. The role reports to the Social Work team and supports discharge planning across inpatient and outpatient services.

You will work with patients, families, and the care team to arrange follow-up care, utilize Medicare forms, and connect patients with housing, transportation, and social services as needed.

Qualifications

• Bachelor's Degree preferred or 3 years of experience; Masters preferred.
• Handle With Care certification within 30 days of hire.
• Requires critical thinking, decisive judgment, and the ability to work with minimal supervision.

Responsibilities

• Communicates with patients, families, and medical staff when arranging follow-up care.
• Keeps track of Medicare forms and consent forms.
• Assists with discharge planning and processes; identifies resources and provides referrals.
• Participates in case reviews to evaluate case management and progress with the healthcare team.
• Interviews and assesses patients or their families and caregivers.
• Determines and arranges needed internal and external services/interventions.
• Assists with group activities.
