Job summary

Encompass Health Corporation in Morgantown, WV is seeking a Case Manager to link high‑quality care from admission to post‑discharge. You will coordinate with an interdisciplinary team, develop discharge plans, and ensure timely access to resources for patients and families.

Responsibilities include assessing goals, documenting case management addenda, and maintaining compliance with regulations while guiding treatment plans and communicating with post‑discharge providers.

Qualifications

• Must be able to independently complete assessments within the scope of practice.
• Active license required or eligibility per state licensure rules.
• CCM or ACM certification required or must be obtained within two years.
• Minimum of 2 years of rehabilitation experience preferred.

Responsibilities

• Work with interdisciplinary team to guide treatment plans based on patient needs.
• Coordinate tentative discharge plan and contingency plans with care team.
• Participate in planning for and execution of patient discharge experience.
• Monitor patient experience: quality, timeliness, service appropriateness, payors, expectations.
• Facilitate weekly team conferences and coordinate treatment plan modifications.
• Complete case management addendums and required documentation.
