Job summary

Saint Clare's Health in New Jersey is seeking a Case Management professional to coordinate discharge planning and support a multidisciplinary team. You will assist with referrals, authorizations, and the development of safe, patient-focused discharge plans.

The role requires knowledge of discharge planning, utilization management, and case management terminology; strong computer skills; and a current BCLS certificate.

Qualifications

• Knowledge of discharge planning, utilization management and case management terminology.
• Experience with basic to intermediate computer skills.
• BCLS (AHA) certificate required upon hire.
• High School diploma required; associates degree or LVN license preferred; hospital experience preferred.

Responsibilities

• Coordinate activities of the Case Management Department under the direction of the Case Manager/Social Worker.
• Assist with discharge planning and development of discharge plans.
• Assist with case management referrals and authorizations.
• Maintain Medicaid state submission forms and file per facility procedures.
• Interview patients and families to develop safe patient-focused discharge plans.
