• Discharge planning, utilization review, case management or healthcare insurance experience.

Responsibilities

• Aids Case Management in securing post-acute care, DME, resources and transportation.
• Coordinates communication with physicians, clinics, and case managers regarding continuum of care.
• Provides timely information to payors, service partners, staff, patients and families.
• Supports discharge planning and placement of the patient with appropriate resources.
• Collaborates with the treatment team to facilitate post-hospitalization services.
