Qualifications

• 2+ years of experience in Discharge Planning and Social Services in a healthcare setting.
• Strong communication and collaboration skills are required.
• 4-year degree in Social Work, Psychology, Sociology, Education, Counseling, Criminal Justice, or related human services field.

Responsibilities

• Coordinate patient discharge including supplies, transportation, and services.
• Schedule discharge meetings and coordinate home visits and therapy.
• Direct five-day discharge planning meetings for all new admissions.
• Assess patient support systems and home situation to inform discharge decisions.
• Facilitate bed management to maximize availability of transitional care services.
• Assist residents with social issues and care coordination.
• Conduct customer satisfaction surveys.
