Novus Group in Pittsburgh, PA is hiring a Clinical Care Discharge Planning Assistant for an on-site, day-shift role. The position offers steady hours, full benefits, and a defined growth path within clinical care coordination.

Starting hourly pay is $21.14–$24.64 with potential progression to $35.13 as experience and performance advance along the career ladder. Qualified applicants have a high school diploma and EMR experience.

Competenze

• Education: High school diploma or GED equivalent required.
• Systems Knowledge & Technical Skills: Basic to proficient computer skills; experience navigating EMR systems and technology programs for chart reviews, data collection, and progress tracking.
• Core Competencies: Excellent written and verbal communication skills; strong organization, time management, and multi-tasking capabilities; ability to take direction, work independently or collaboratively, and solve operational problems.
• Preferred Experience: Prior healthcare work experience, case management support, or discharge planning familiarity preferred.

Mansioni

• Discharge logistics: perform non-clinical tasks to support safe discharge plans.
• Patient and family engagement: serve as admin contact, provide required educational forms, resources, and discharge documentation.
• EMR documentation: review charts, track interdisciplinary communications, document progress, and report discharge barriers or care plan concerns promptly to clinical teams.
• Departmental operations & compliance: perform clerical duties, assist with internal audits, and maintain HIPAA compliance.
