The position focuses on discharge planning within an interprofessional framework and coordination with community providers to determine the next site of care. The ideal candidate has a Bachelor's degree in a related field or an Associate degree with QHP, with strong writing and communication skills and experience in a healthcare setting.

Qualifications

• Bachelor's degree in related field required, or associate degree in related field with QHP required.
• Experience in healthcare/medical setting preferred.
• Strong writing and communication skills required.
• Experience with diverse populations and multicultural issues preferred.

Responsibilities

• Provides discharge planning as a member of an interdisciplinary team.
• Provides discharge planning, referrals, coordination with community providers and provision of other treatment related duties.
• Responsible for patient progression of care, interviewing patients for initial discharge plans and coordinating care to the next appropriate site of care.
