Job summary

Oceans Behavioral Hospital Amarillo is seeking a Discharge Planner to coordinate discharge activities for inpatient programs. You will collaborate with physicians, therapists, and nursing staff to ensure smooth transitions and compliant documentation.

The role requires an associate degree (healthcare) or a bachelor’s in social services, and at least one year of psychiatric/chemical dependency experience. Strong communication and organizational skills are essential. BLS certification is required.

Qualifications

• Associate or bachelor’s degree in a healthcare/social services field.
• At least one year psychiatric/chemical dependency experience preferred.
• Must complete and maintain Basic Life Support (BLS) certification and Oceans-approved behavioral health de-escalation program.

Responsibilities

• Coordinate discharge planning with physicians, therapists, and nursing staff.
• Review psychosocial data and discharge assessments for treatment teams.
• Schedule follow-up appointments and communicate with families.
• Document discharge planning activities and ensure timely completion of CQI forms.
• Complete referral processes and arrange post-discharge care.
