Job summary

North Alabama Medical Center in Florence, AL is seeking a Discharge Planner to join our Case Management team. You will coordinate patient discharge planning, arrange follow-up care, and work with families and healthcare staff to ensure smooth transitions home or to next care settings.

The role requires a Bachelor's Degree (preferred) or equivalent experience, strong communication, and the ability to function under pressure. This is a full-time position at our Florence campus.

Qualifications

• Bachelor's Degree preferred or 3 years of experience in discharge planning.
• Strong critical thinking and decision-making abilities required.
• Ability to work with minimal supervision in a high-stress environment.

Responsibilities

• Communicates with patients, families and healthcare staff to arrange follow-up care.
• Keeps track of Medicare forms and consent forms.
• Assists with discharge planning and identifies resources (transportation, housing, social services).
• Participates in case reviews to monitor progress and ensure compliance with treatment plans.
• Interviews patients or families to assess needs and coordinates services.
