Job summary

North Alabama Medical Center in Florence, AL is seeking a Discharge Planner to coordinate patient transitions from hospital to home or another care setting. You will partner with case management, physicians, and care teams to develop individualized discharge plans and ensure timely follow-up.

The role requires strong communication, organizational skills, and the ability to work under pressure. Bachelor's degree preferred or equivalent experience, with comprehensive benefits and growth

Qualifications

• Bachelor's degree preferred or 3 years of experience in discharge planning or case management.
• Experience in healthcare setting is a plus.

Responsibilities

• Communicates effectively with patients and families to arrange follow-up care.
• Keeps track of Medicare forms and consent forms.
• Assists with discharge planning and identifies resources for the discharge plan.
• Participates in case reviews to evaluate progress and compliance with treatment plans.
• Interviews and assesses patients or family, caregivers, or legal representatives.
• Determines and arranges needed internal and external services/interventions.

Discharge Planner Position Summary

Discharge planning is the process of transitioning a patient from one level of care to the next. Ideally, discharge plans are individualized instructions provided to the patient as they move from the hospital to home or instructions provided to subsequent healthcare providers. Discharge planner reports off to the Case Management team and staff.

Reports to: Director of Case Management

FLSA: Non-Exempt

Education: Bachelor's Degree preferred or 3 years of experience;

Licenses: None Listed

Required Skills

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

Essential Functions

• Communicates effectively and routinely with patients, families, medical staff, nursing staff, other caregivers when arranging follow-up care
• Keeps track of all Medicare forms and consent forms
• Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan
• Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan
• Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives
• Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions
• Assisting with group activities
