Job summary

David Lawrence Center in Naples, Florida, is seeking a full-time, non-exempt role blending clinical skills with case management. The position starts at $20.00 per hour, with comprehensive benefits and ongoing professional development opportunities.

Expect 40-hours with occasional rotating weekend coverage during standard business hours. The role focuses on discharge planning, referrals, and collaboration with psychiatrists, nurses, and community partners to support adults and families in crisis

Qualifications

• Bachelor's Degree in Clinical Psychology, Social Work, Behavioral Science, Rehabilitation Counseling or other relevant field.
• At least a year of related or relevant experience.
• Ability to function at a high level in a demanding environment.
• Knowledge of community services and resources preferred.
• A 'How can I help?' attitude when it comes to working across programs.
• Ability to relate to a wide variety of co-workers, clientele, and community agencies.
• Accurate, organized and informative record-keeping/reporting skills.
• Ability to ensure that all activities are managed according to policy and procedure.
• Microsoft Office 365 skills (Including Teams/Video Conferencing) and EMR experience.
• Staying compliant on trainings and immunizations.
• Ability to pass Level II DCF Mental Health Fingerprinting, Drug Screen and Local Criminal Check.

Responsibilities

• Facilitating timely, thoughtful and accurate discharges for Clients admitted to CSU/CCSU.
• Coordinating referrals and service linkages.
• Communicating with local community resources related to Mental Health and Substance Use Disorders.
• Comprehensive care coordination with psychiatrists, nurses, technicians, external providers, and families.

Job Summary:

DLC's Acute Care Program provides high-impact, transformative and imperative service to those who might not otherwise have access to rehabilitation resources. This is an opportunity that combines clinical skills with case management experience and works under a truly dynamic Management Team.

Key Responsibilities:

• Facilitating timely, thoughtful and accurate discharges for Clients who have been admitted to the Adult and Children Crisis Stabilization Unit (CSU/CCSU).
• Coordinating referrals and service linkages.
• Communicating with local community resources related to Mental Health and Substance Use Disorders.
• Comprehensive care coordination working with psychiatrists, psychiatric nurses, technician staff, external providers, family members, etc.

Physical Requirements: This position requires a mix of physical activities throughout the workday. Duties involve periods of standing, sitting, and walking, often across various areas of the site. The role also includes tasks that may require bending, reaching, and other physical movements. Candidates must be able to occasionally lift and carry items weighing up to 50 pounds. A reasonable level of physical stamina and mobility is necessary to successfully perform the essential functions of this role.
