Job Summary:

DLC's Acute Care Program provides high-impact, transformative and imperative service to those who might not otherwise have access to rehabilitation resources. This is an opportunity that combines clinical skills with case management experience and works under a truly dynamic Management Team.

Key Responsibilities:

• Facilitating timely, thoughtful and accurate discharges for Clients who have been admitted to the Adult and Children Crisis Stabilization Unit (CSU/CCSU).
• Coordinating referrals and service linkages.
• Communicating with local community resources related to Mental Health and Substance Use Disorders.
• Comprehensive care coordination working with psychiatrists, psychiatric nurses, technician staff, external providers, family members, etc.

• Bachelor's Degree in Clinical Psychology, Social Work, Behavioral Science, Rehabilitation Counseling or other relevant field = Required
• At least a year of related or relevant experience = Required
• Ability to function at a high level in a demanding environment = Required
• Knowledge of community services and resources = Preferred

• Ability to relate to a wide variety of co-workers, clientele, and community agencies = Required

Physical Requirements: This position requires a mix of physical activities throughout the workday. Duties involve periods of standing, sitting, and walking, often across various areas of the site. The role also includes tasks that may require bending, reaching, and other physical movements. Candidates must be able to occasionally lift and carry items weighing up to 50 pounds. A reasonable level of physical stamina and mobility is necessary to successfully perform the essential functions of this role.
