Job summary

The Pearl at Fort Lauderdale Rehabilitation and Nursing Center is seeking a Discharge Planner to ensure seamless transitions from the facility to the next destination. You will collaborate with care teams, patients, and families to develop individualized discharge plans.

Responsibilities include coordinating discharge activities, maintaining relationships with providers, ensuring regulatory compliance, and contributing to quality improvement initiatives.

Qualifications

• 1 year of experience in discharge planning or related field.
• High school diploma or equivalent; degree in a related field preferred.
• Strong communication, interpersonal, and problem-solving skills.

Responsibilities

• Coordinate discharge planning activities, assessing patient needs and barriers.
• Develop and maintain relationships with patients, families, and providers.
• Collaborate with interdisciplinary teams to address issues and plan care transitions.
• Stay updated on regulations and guidelines affecting discharge planning and ensure compliance.
• Participate in quality improvement initiatives to improve processes and outcomes.
• Maintain accurate and timely records of discharge plans and progress.
