Job summary

The Pearl at Fort Lauderdale Rehabilitation and Nursing Center seeks a Discharge Planner to coordinate patient transitions from our care setting to their next destination. You will work with patients, families, and clinical teams to tailor and implement effective discharge plans, ensuring continuity of care and favorable outcomes.

Key responsibilities include collaborating with interdisciplinary teams, addressing barriers, maintaining accurate records, and staying current with discharge planning

Qualifications

• 1 year of experience in discharge planning or a related field.
• High school diploma or equivalent; degree in a related field preferred.
• Strong communication, interpersonal, and problem-solving skills.

Responsibilities

• Coordinate discharge planning activities, assess patient needs, identify barriers, and develop intervention strategies.
• Develop and maintain relationships with patients, families, and healthcare providers to support informed decisions and care transitions.
• Collaborate with interdisciplinary teams to address issues and optimize patient outcomes.
• Stay up-to-date with regulations and ensure compliance with applicable guidelines.
