Qualifikationen

• Registered Nurse or LPN preferred.
• Discharge planning and care coordination experience preferred.
• Experience in hospital discharge transitions and inter-organizational collaboration.

Aufgaben

• Assess discharge planning needs through review of medical records, patient interviews, and evaluation of factors affecting post-discharge care.
• Develop and coordinate individualized discharge plans aligned with physician orders, payer requirements, and available community resources.
• Collaborate with hospitalists, nursing staff, case managers, therapists, pharmacists, social workers, and ancillary departments to coordinate timely, safe discharges.
• Arrange post-acute services including home health, SNFs, rehabilitation services, durable medical equipment, hospice, and transportation.
• Educate patients and caregivers regarding discharge instructions, medications, follow-up, disease management, warning signs, and community resources.
• Identify barriers to discharge and coordinate interventions such as insurance authorization, transportation, housing, and social determinants of health.
• Communicate with primary care providers, specialists, and post-acute providers to facilitate continuity of care and reduce readmissions.
• Document assessments and transition plans in the EHR per organizational standards and regulatory requirements.
• Monitor discharge timelines and participate in quality initiatives to improve care transitions.
