Job summary

Family & Children’s Services in Oklahoma City is seeking a discharge coordination and capacity management role to ensure seamless transitions for consumers moving from the Urgent Recovery Center (C3) into Hope outpatient services or other appropriate care, and to monitor hospital bed availability across the community.

You will partner with team members to maintain bed/chair capacity at the Urgent Recovery Center, and coordinate client transfers to other MHC locations or mental health agencies as

• Must possess a valid driver's license and satisfactory driving record.
• Skills and competence to establish supportive trusting relationships with persons with mental illnesses and/or substance abuse issues.

Job description

Responsible for (1) ensuring a seamless transition for consumers discharging from Urgent Recovery Center (C3) into the care of Hope outpatient services, another outpatient agency or another appropriate level of care (2) Responsible capacity management of chairs in the Urgent Recovery Center (C3) ,beds in the Urgent Recovery Center and community wide hospital bed availability. Work in partnership with other team members for continuation of service in case of client transfer to other MHC locations or with other mental health agencies.

Requirements

Bachelor’s Degree in the social sciences field is preferred, with consideration given for degrees in Criminal Justice or Education. Must have a strong commitment to the right and ability of each person served to live in normal community residences; and have access to helpful, adequate, competent, and continuous supports and services. Skills and competence to establish supportive trusting relationships with persons with mental illnesses and/or substance abuse issues and respect for client rights and personal preferences in treatment are essential. Must possess a valid driver's license and satisfactory driving record and use personal automobile to travel to locations other than primary office.
