Job summary

Acadia Healthcare is seeking a discharge planning professional to develop ongoing care plans and coordinate post-discharge services in a behavioral health setting.

The role requires clinical experience and licensure (RN/LPN) with CPR certs. You will coordinate appointments, counseling services, and community resources for a smooth transition from the facility.

Qualifications

• Associates degree in nursing or human services; bachelor’s degree preferred.
• RN/LPN licensure in the state where the facility provides services.
• CPR certification required; de-escalation certification may be required.
• First aid may be required based on state or facility requirements.
• One or more years’ experience in clinical setting and discharge planning preferred.

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:

• Associates degree in nursing or human services field required. Bachelor's degree in social work or human services field preferred.
• One or more years’ experience in a clinical setting preferred.

• One or more years’ experience in discharge planning preferred.

LICENSES/DESIGNATIONS/CERTIFICATIONS:

• If applicable, current licensure as a LPN, RN within the state where the facility provides services.
• CPR and de-escalation and restraint certification required (training available upon hire and offered by facility.
• First aid may be required based on state or facility requirements.
