Job summary

Acadia Healthcare in Chattanooga, TN is seeking a discharge planning professional to develop aftercare plans and coordinate post-discharge follow-up across behavioral health services. You will arrange appointments, connect patients with community resources, and ensure documentation supports the continuum of care.

Qualification includes an associate degree in nursing or human services, with a bachelor's preferred, and 1+ year experience in clinical settings and discharge planning.

PURPOSE STATEMENT:

• Act as a liaison between referral sources, patients and counselors.
• Ensure all patient discharge planning documentation is clear, concise and timely.

OTHER FUNCTIONS:

• Perform other functions and tasks as assigned.

Qualifications

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:

• Associates degree in nursing or human services field required. Bachelor's degree in social work or human services field preferred.
• One or more years’ experience in a clinical setting preferred.

• One or more years’ experience in discharge planning preferred.

LICENSES/DESIGNATIONS/CERTIFICATIONS:

• If applicable, current licensure as a LPN, RN within the state where the facility provides services.
• CPR and de-escalation and restraint certification required (training available upon hire and offered by facility.
• First aid may be required based on state or facility requirements.
