Job summary

Qualifications

• Must be licensed in Nebraska as an RN or hold Nebraska licensure as an MSW with MSW from an accredited program.
• RN candidates: BSN preferred and experience in acute care, case management, or discharge planning.
• MSW candidates: MSW required; clinical social work licensure preferred where applicable.

Responsibilities

• Develops and implements discharge planning services aligned with hospital goals.
• Coordinate with patients, families, and care team to ensure safe transitions of care.
• Maintain communications with caseworkers and community resources to support referrals.
• Coordinate transportation and post-discharge arrangements for patients.
• Ensure documentation complies with regulatory standards and HIPAA.
