Job summary

Fairview Skilled Rehabilitation & Nursing Center seeks a compassionate Discharge Planner / Social Worker to coordinate safe discharges for residents, working with families, physicians, nursing, therapy, and community providers to ensure smooth transitions.

The role includes psychosocial assessments, arranging home care and services, and maintaining compliant documentation while collaborating in care plan meetings with the interdisciplinary team.

Qualifications

• MSW degree or higher preferred.
• LMSW/LCSW licensure preferred.
• Experience with discharge planning in related settings.
• Knowledge of Medicare/Medicaid procedures and regulations.

Discharge Planner / Social Worker

Fairview Skilled Rehabilitation & Nursing Center is seeking a compassionate and organized Discharge Planner / Social Worker to coordinate safe and timely discharges for our residents. This position works closely with residents, families, physicians, nursing, therapy, and community providers to ensure a smooth transition from our facility to the next level of care.

Responsibilities

• Coordinate discharge planning from admission through discharge.
• Complete psychosocial assessments and provide emotional support to residents and families.
• Arrange home care, medical equipment, transportation, and community services.
• Communicate with hospitals, physicians, insurance companies, and outside providers.
• Maintain accurate documentation and ensure compliance with state and federal regulations.
• Participate in care plan meetings and collaborate with the interdisciplinary team.

Qualifications

• MSW preferred; LMSW/LCSW preferred.
• Experience in skilled nursing, rehabilitation, hospital, or case management preferred.
• Strong communication, organizational, and teamwork skills.
• Knowledge of Medicare, Medicaid, and discharge planning is a plus.
