Job summary

North Canyon Network of Care is seeking a Discharge Planner to identify patient needs and priorities for care through assessment, planning, intervention and evaluation. You will coordinate care between professional disciplines and provide psychosocial support to patients and families facing chronic, acute, or terminal illness.

The role involves advising family caregivers, counseling patients, and helping plan for discharge needs to ensure smooth transitions of care.

Qualifications

• Excellent written and verbal communication skills.
• Current BLS certification.
• ACLS and PALS recommended but not required.
• Ability to work with patients across growth and development stages.
• Ability to manage chaotic environments and changing patient needs.
• Demonstrates behaviors aligned with service excellence and organizational values.

Responsibilities
