Kids for the Future is seeking a Discharge Planner for an on-site role in San Diego. You will assess discharge needs, coordinate planning, and work with the treatment team to ensure continuing care and community supports for residents.
Qualifications include a BA/BS in Psychology, Social Work, or Counseling, CPR/First Aid certification, and 4+ years in behavioral health. PTO accrues, and PSLF eligibility is available.
40 hours per week; Monday - Friday, 8:00 am to 4:30 pm. This is an on-site position and requires in-person attendance to perform the essential functions of the job.
Our Crisis Centers are Short Term Acute Residential Treatment (START) programs (Del Sur Crisis Center, Esperanza Crisis Center, Halcyon Crisis Center, Hawthorne Crisis Center , Jary Barreto Crisis Center, New Vistas Crisis Center , and Turning Point Crisis Center ) that provide intensive, community-based crisis residential services for adults throughout San Diego County. They deliver a unique alternative to psychiatric hospitalization to clients who are experiencing a psychiatric crisis of such magnitude so as not to be manageable on an outpatient basis. Their services are provided in a home-like environment that specializes in the psychosocial rehabilitation of each individual.

Bachelor's degree in Psychology, Social Work, or Counseling required
Provides psychosocial rehabilitation interventions and services in order to meet the clinical needs of clients. Assists clients in accessing and using medical care, community resources, family and other support services to achieve an optimal level of health and functioning in the community.
