Job summary

AHMC Anaheim Regional Medical Center is seeking a Discharge Planner to coordinate patient discharge planning across hospital areas, aligning with customer service expectations and ongoing care needs. You will work with patients, families, and the interdisciplinary team to assess needs and arrange appropriate services.

The role emphasizes community resource navigation, documentation, and advocacy for patient rights, with preferred qualifications in social work or nursing, and at least two years

Qualifications

• Bachelor’s degree from an accredited school of Social Work or related field (preferred), RN or LVN (preferred)
• Two years acute or related medical care experience.

Responsibilities

• Consistently applies infection control policies/practices.
• Meets population specific competencies.
• Attends department specific education/training, inservices, staff meetings.
• Department specific performance improvement project.
• Demonstrates ability to provide timely and appropriate discharge planning assessments and interventions for patients .
• Demonstrates ability to develop and implement appropriate discharge planning plan of care.
• Demonstrates ability to advocate for patient’s rights.
• Knowledgeable regarding community resources and demonstrates ability to mobilize them on patient’s behalf.
• Demonstrates ability to maintain appropriate documentation and statistical records.
