Job summary

UAB Medicine is seeking a Patient Care Technician to partner with nursing and care transition teams to streamline patient discharges and coordinate post-discharge needs. The role involves assisting with activities of daily living, taking vitals, and ensuring safe transport arrangements as patients await discharge completion.

Qualifications

• High school diploma or equivalent; five years of patient care experience required.
• CPR certification required or must be obtained within 30 days of employment.

Responsibilities

• Coordinate discharge planning with Nursing Resource Coordinator, Care Transitions team, and units to facilitate early discharges.
• Assist with ADLs and vital signs as needed; record vitals, weights, inputs/outputs.
• Coordinate transportation and other services (ambulance, nutrition) to support discharge lounge activities.
