You will coordinate patient discharge transportation using ambulances, wheelchair vans, taxis, and other options, balancing patient needs, insurance requirements, and availability to support efficient discharge planning and a positive patient

Qualifications

• Education: High school diploma/GED.
• Experience: Relevant experience in the field.

Responsibilities

• Coordinating patient discharge transportation using ambulances, wheelchair vans, taxis, buses, and other transportation resources.
• Communicating with patients, families, clinical teams, case managers, and transportation vendors to arrange appropriate transport solutions.
• Navigating insurance coverage requirements and transportation eligibility guidelines to support efficient discharge planning.
• Prioritizing transportation requests while managing multiple cases simultaneously in a dynamic healthcare environment.
• Investigating transportation concerns, resolving service issues, and supporting positive patient experiences.
• Tracking transportation activity, processing invoices, and maintaining accurate records in accordance with departmental procedures.
• Supporting process improvement initiatives and assisting with training and onboarding of new team members.
