Job summary

USC Verdugo Hills Hospital is seeking an experienced administrative professional to support the Case Management team with discharge planning and patient communication. You will collaborate with Case Managers and Assistants to ensure discharge plans are clearly conveyed to patients and families, contributing to a positive discharge experience.

Responsibilities include post-discharge follow-ups to confirm post-acute services are arranged and that patients are satisfied with the discharge process

Qualifications

• High school graduate or equivalent; some college and/or business education with emphasis on English.
• Office skills preferred; LVN preferred.
• Minimum of four years secretarial experience.
• Typing 40-55 WPM; experience with data entry.
• BLS Certification; BLS/ACLS/PALS/NRP active at hire.

Responsibilities

• Provide department support for Case Management to facilitate discharge planning.
• Work with Case Managers and Assistants to communicate plans to patients and families.
• Follow up post-discharge to ensure post-acute services are provided as ordered.
• Ensure patient experience with discharge process is positive.
