Job summary

University of Southern California (Keck Medicine of USC) seeks a capable administrative professional to support the Case Management team with discharge planning and throughput. You will communicate discharge plans to patients and families, helping to ensure a smooth transition and positive experience.

The role includes post-discharge follow-up to verify post-acute services are provided as ordered and patient satisfaction is achieved, working with Case Managers and Case Management Assistants

Qualifications

• High school diploma or equivalent; some college or business education with emphasis on English.
• Four years secretarial experience; typing 40-55 WPM; experience with computer data entry.
• BLS Certification; BLS, ACLS, PALS, and NRP must be AHA certified; active at hire/transfer date.

Responsibilities

• Provide department support for the Case Management team to facilitate discharge planning and throughput.
• Communicate discharge plans to patients and families to support a positive patient experience.
• Follow up post discharge to ensure post-acute services are provided as ordered and patient satisfaction is maintained.
