Job summary

Centra Behavioral Health Hospital, part of Lifepoint Health, seeks a Discharge Coordinator to manage aftercare for patients and coordinate resources post-discharge. The role supports treatment planning by arranging follow-up appointments and providing families with relevant care information.

Applicants should have a bachelor’s degree in social work, counseling, or psychology, or an equivalent state license, with CPR and CPI within 30 days. This is a PRN position at our new Lynchburg facility.

Responsibilities

• Provide aftercare coordination for patients needing ongoing medical/psychosocial care after discharge.
• Create and maintain up-to-date aftercare coordination resources.
• Support treatment planning by arranging aftercare appointments with providers and therapists.
• Provide resource information to patients and families for future care needs.
• Communicate proactively with clients, families, aftercare providers and referral sources.
• Create a detailed Discharge Summary and Contract for each patient.

How You'll Contribute

A Discharge Coordinator who excels in this role:

• Provides aftercare coordination for patients who need ongoing medical/psychosocial care following discharge.
• Creates and maintains up to date aftercare coordination community resources.
• Displays active involvement in treatment planning process, by setting up the aftercare appointments as identified by the patients' provider and therapist.
• Provides resource information to patients and families as appropriate for future care needs.
• Actively communicates with clients, families, aftercare providers, and referral sources. Demonstrates proactive communication with those involved with the patients' treatment.
• Creates a detailed Discharge Summary and Contract for each patient as assigned.

What We're Looking For

Applicants should have a bachelor's degree in social work, counseling, psychology, or relevant state licensure or at least three years of clinical experience within an inpatient behavioral health setting. Additional requirements include:

• Prior experience with psychiatric and chemical dependency patients.
• Unencumbered clinical license if eligible/needed per state regulations, preferred.
• CPR certification and Crisis Prevention Training (CPI) within 30 days.
