Job summary

Centra Behavioral Health Hospital in Lynchburg, VA, part of Lifepoint Health, is opening a brand-new 72-bed behavioral health facility and seeking an Assessment Specialist for a full-time night shift. The role includes initial assessments, referrals, and level-of-care determinations with thorough documentation and coordination with physicians.

Qualifications

  • Master's degree in social work or counseling, or nursing license.
  • Experience in psychiatric care; chemical dependency, dual diagnosis, geriatric experience preferred.
  • Experience in patient assessments, treatment planning, and external review coordination.
  • Current unencumbered clinical social work, counseling license, or RN per state guidelines.
  • CPR certification and CPI training within 30 days of employment and before patient contact.
  • Must be at least 21 years old; may require flexible hours.

Responsibilities

  • Facilitates response to requests for services, assessment, referrals, and level of care.
  • Conducts initial evaluation, documents symptoms/needs, and informs MD for LOC.
  • Delivers excellent customer service to patients, physicians, visitors, and coworkers.
  • Maintains preadmission/admission paperwork and completes pre-certifications with insurers.
  • Monitors patient condition and notifies physician as needed.
  • RN follows hospital policy for medication administration.
  • Exhibits empathy, dignity, and respect in interactions.
  • Assists with comprehensive nursing assessments.

How You'll Contribute

An Assessment Specialist who excels in this role:

  • Facilitates response to a request for services, including the initial response, the assessment, the referral process, the designation of the appropriate level of care, admission into services, and appropriate follow‑up activity.
  • Conducts initial evaluation, initiates the assessment, documents patient symptomology, functioning, and needs, and provides information to MD for LOC determination.
  • Demonstrates excellent customer service skills in assisting patients, physicians, visitors, and coworkers promptly and courteously.
  • Possess clear knowledge of Emergency Detention paperwork and processes and EMTALA.
  • Maintains all preadmission and admission paperwork with accuracy and completes all pre‑certification with insurance companies.
  • Will monitor patient for change of condition and respond accordingly up to and including notification of the physician.
  • RN will follow hospital policy for medication administration.
  • RN will consistently use empathy, dignity, and respect when interacting with patients.
  • Assists/performs comprehensive nursing assessments on all patients.

What We're Looking For

Applicants should have a master's degree in social work or counseling, or graduate from an accredited program of professional nursing or relevant state licensure required. Additional requirements include:

  • Previous experience in a psychiatric health care facility, with direct experience working with chemical dependency, dual diagnosis, psychiatric and geriatric patients, preferred.
  • Experience in patient assessments, family motivations, treatment planning and communication with external review organizations or comparable entities.
  • Current unencumbered clinical social work, counseling license, Registered Nurse or per state of practice guidelines.
  • CPR certification and Crisis Prevention Training (CPI) within 30 days of employment and prior to any patient contact.
  • Must be at least 21 years of age. May be required to work flexible hours and overtime.
