Job summary

You will coordinate care packages, perform Care Act assessments, and work with inpatient wards, community teams and housing services to ensure safe, timely transitions and outcomes focused on recovery and independence.

Qualifications

• HCPC registration is required.
• Experience with Care Act assessments and safeguarding.
• Experience with discharge planning and care coordination.
• Ability to work within a multidisciplinary team.

Responsibilities

• Lead social care aspects of admission, care planning and discharge.
• Coordinate packages of care and post-discharge support.
• Conduct Care Act assessments and reviews.
