Job summary

Liquid Personnel is recruiting a Social Worker for its Adults Discharge to Assess Team in Bristol. This role focuses on supporting citizens during transitions from hospital to community settings, ensuring safe and effective discharge planning.

Qualifications

• Qualified Social Worker with Social Work England registration.
• Experience in Adults Services, ideally discharge or hospital-based teams.
• Strong knowledge of Care Act, MCA, and safeguarding legislation.
• Ability to work in a fast-paced environment and manage competing priorities.

Responsibilities

• Assess and support citizens on discharge from hospital across three discharge pathways.
• Complete post-discharge assessments and reviews in line with the Care Act.
• Undertake Care Act Assessments (CAA) and support planning.
• Carry out Best Interest decisions and work within the MCA.
• Manage safeguarding concerns in line with statutory responsibilities.
• Provide duty cover where required.
• Attend QA panels to support funding decisions.
• Flexible working options where available.
