Job summary

Methodist Dallas Medical Center is seeking a Care Transitions Navigator to coordinate activities that promote quality outcomes, patient throughput, and discharge planning while balancing care with resource use.

The role identifies barriers to throughput, minimizes delays in discharge plans, and works with care teams to ensure smooth transitions for patients. A degree in Social Work or nursing background is preferred, with licensure as LMSW/LBSW or RN BSN preferred.

Qualifications

• Bachelor's degree in Social Work, Master's in Social Work, or RN with BSN preferred.
• LMSW/LBSW or RN licensed by the Texas Board preferred.
• CCM or ACM certification preferred.
• 1 year related work experience.
• Ability to prioritize multiple tasks in a fast-paced work environment.
• Ability to periodically flex work schedule as indicated by client or hospital needs.
