Job summary

Methodist Southlake Medical Center is seeking a Care Management leader to oversee day-to-day operations and ensure appropriate resource use aligned with quality outcomes. The role focuses on staff development and guiding Care Transition Navigators within the Care Management Department.

Required qualifications include a Master’s in Social Work or a BSN with an LMSW or RN license in Texas, plus a minimum of five years of clinical practice.

Qualifications

• Master’s degree from an accredited school of social work or BSN from an accredited school of nursing.
• LMSW as licensed by the Texas State Board of Social Worker Examiners or RN license by the Texas Board of Nursing.
• A minimum of five years of clinical practice.

Responsibilities

• Oversee day-to-day operations of the Care Management Department.
• Assure appropriate resource utilization related to patient care with quality outcomes.
• Retain and develop competent staff.
• Assist in planning, directing and implementing operational, financial and personnel activities of Care Transition Navigators.
• Provide guidance in prudent practice.
