Job summary

UMass Memorial Health in Worcester, MA is seeking a Discharge Planner/Case Manager to join the Case Management team. You will coordinate daily discharge planning with clinical teams, update EDDs and placement details, and ensure seamless transitions for patients.

Applicants should have a Bachelor's degree, 1–3 years in discharge planning or care coordination, and an active BLS certification or within 30 days of hire.

Qualifications

• Bachelor's degree required.
• 1–3 years experience in discharge planning, care coordination support, or related health science role.
• Active BLS certification or within 30 days of hire.

Responsibilities

• Support daily discharge planning by coordinating and communicating with care teams, updating Estimated Discharge Date (EDD) and placement details.
• Initiate, coordinate, and track referrals and transfers with required documentation.
• Complete essential discharge workflows and regulatory tasks (Medicare notices, compliance).
• Coordinate post-acute services such as rehab, SNF placement, VNA, DME, and MassHealth forms.
• Identify and escalate discharge barriers (insurance, transportation, placement delays).
• Maintain relationships with post-acute providers and up-to-date resource lists for care teams.
• Facilitate access to behavioral health, housing supports, and community resources.
• Assist with insurance coordination, prescriptions support, form preparation, and notary needs.
