Qualifications

• Required: Current Massachusetts Nursing License.
• Graduate of an accredited school of nursing, RN or BSN.
• Experience in discharge planning and utilization review.
• Equivalency of education and experience may be substituted per policy.

Responsibilities

• The Care Manager is a licensed health care professional responsible for utilization review and discharge planning.
• Collaborates with physicians, nurses, social workers, and other health-care team members.
• Maintains practice patterns per state licensure and Joint Commission Standards.
• Adheres to professional care management ethical standards.
• Demonstrates knowledge of discharge planning and bed status/insurance authorization processes.
• Ensures timely communication with health plan case managers regarding ECC policies.
