UPMC Memorial in York is seeking a Manager to lead the Clinical Care Coordination and Discharge Planning team. This full-time day shift role has no weekends or holidays.

The Manager will oversee daily operations, mentor staff, and ensure compliant documentation and budgeting under the Director's direction. The position requires a Master's degree with at least five years of discharge planning or care coordination experience and leadership responsibilities.

Qualifications

• Master's degree required with at least five years of discharge planning or care coordination experience, including leadership.
• Licensure: RN, LCSW, or education-appropriate license required; CCM/ACM, LCSW or UPMC approved certification preferred.
• Experience managing departmental operations and supervising staff including evaluations and performance feedback.

Responsibilities

• Collaborates with the Director to ensure effective processes and systems to meet departmental goals.
• Oversees daily operations of the Discharge Planning staff and ensures productivity standards.
• Directs discharge planning coordination among health care providers and optimizes resource use.
• Hires, terminates, and evaluates staff performance; conducts annual evaluations.
• Serves as a resource for care management and denial management; ensures compliant documentation of data.
• Develops and maintains training programs for department staff.
• Maintains a tracking system to monitor progress toward goals and objectives.
• Works with other UPMC departments to address discharge planning issues and report trends to the Director.
• Contributes to annual department budget development and monitors expenses against the budget.
