Qualifications

• Experience with hospital discharges and community integration preferred.

Responsibilities

• Respond to admission notifications 24/7 on a rotating basis.
• Travel to inpatient units to engage with clients post-admission.
• Obtain signed releases and coordinate follow-up care with outpatient providers.
• Conduct brief psychosocial intake and document in EMR.
• Liaise with clients’ outpatient providers and share discharge information.
• Maintain daily telephonic contact with inpatient units and coordinate post-discharge visits.
