Job summary

The Case Assistant at Carle Health supports discharge planning under the direction of the RN Case Manager and Social Worker, coordinating referrals, transportation, and authorization requests to ensure a smooth transition for patients.

You will help maintain Medicare MOON compliance, interact with patients and families, and document activity in Epic to support accurate records and post‑acute care planning.

Qualifications

• Certified nurse assistant required.
• Experience collaborating with RN Case Managers and Social Workers.
• Knowledge of Medicare MOON guidelines and discharge processes.

Responsibilities

• Collaborate with the RN Case Manager and SW to develop a plan of care for discharging the patient.
• Assist with nursing home placement and referrals to ensure smooth transitions.
• Maintain Medicare regulatory compliance with the Important Message documents.
• Schedule post-discharge appointments with primary and specialty providers, in and out of network.
• Document activities in Case Management system and Epic.
• Obtain prior authorizations as needed.
• Act as liaison for ED providers and hospitalists for ancillary services.
