Job summary

Qualifications

Responsibilities

• Coordinate, communicate and facilitate clinical progression of discharge plans.
• Collaborate with patients, families and multidisciplinary teams to meet goals.
• Oversee interfacility transitions and handoffs between acute and post-acute care.
• Document and educate about discharge options and resources.
