Job summary

Hackensack Meridian Health seeks a Supervisor, Care Management to oversee care management, utilization management and discharge planning across the team. You will coordinate patient care across the continuum, ensure adherence to evidence-based standards, and drive performance improvements.

The role requires NJ RN or LSW/LCSW licensure, 3+ years in acute care, and strong leadership. Expect collaboration with physicians, hospitalists, and ancillary departments within a comprehensive benefits

Qualifications

• RN license in New Jersey or licensed social worker with NJ credentials.
• 3+ years of full-time experience in an acute care setting.
• Experience with care coordination, case management or discharge planning.
• Strong decision-making, problem-solving and leadership abilities.
• Proficient in Microsoft Office and/or Google Suite.

Responsibilities

• Oversee care management team and caseloads to meet staffing needs.
• Coordinate patient care across the continuum and remove barriers to timely care.
• Monitor LOS, throughput, and patient flow; address denials and appeals.
• Lead and participate in multidisciplinary rounds and performance improvement.
• Present metrics, participate in regulatory audits and ensure regulatory compliance.
