Job summary

Hackensack Meridian Health is seeking a Care Coordinator in Care Management to coordinate and facilitate the clinical progression of patients’ treatment and discharge plans. You will be accountable for a designated patient caseload and work with families and multidisciplinary teams to meet treatment goals and arrange the appropriate next level of care.

Responsibilities include overseeing interfacility transitions and ensuring smooth handoffs between acute and post-acute services.

Qualifications

• Must-have advanced decision-making and problem-solving abilities.
• Strong written and verbal communication skills.
• Proficient computer skills; familiarity with MS Office/Google Suite.
