Job summary

Meadowwood Hospital seeks a Case Manager Coordinator to coordinate discharge planning and ensure continuity of care across the treatment team. You will advocate for patient needs and support safe transitions while complying with regulatory standards.

You will review initial information, develop care plans, and liaison with insurance providers and the business office as needed to optimize patient outcomes.

Qualifications

• Bachelor’s degree in Health Administration, Social Science or related field.
• Ability to liaise with community agencies to ensure continuity of care.
• Knowledge of mental health and substance use is preferred.

Responsibilities

• Ensure admission criteria are met and review information from admission to discharge.
• Develop and monitor individual care plans; collect data and review documentation.
• Coordinate care by tracking and reporting on inpatient/outpatient services.
• Apply knowledge to coordinate cases for the patient population at the facility.
• Share medical criteria between insurance providers and the treatment team as needed.
• Perform precertification with payers and negotiate approved days.
• Assign tasks for admission/review and participate in discharge planning.
