Qualifications

• Bachelor's degree in Health Administration or Social Science or related field.
• Ability to create and maintain liaison with community agencies to ensure continuity of care.
• Knowledge of mental health and substance use fields is preferred.

Responsibilities

• Coordinate care by tracking, monitoring, and reporting on inpatient or outpatient care.
• Develop and implement individual care plans, collecting clinical data.
• Conduct precertification with third party payers and negotiate approved days.
