Qualificações

• Proof of successful completion of education requirements for registered nurse and licensure in good standing.
• BLS required.
• Preferred have at least 5 years’ experience as a Registered Nurse.
• Advanced clinical expertise in inpatient care and complex disease processes.

Responsabilidades

• Coordinate care for patients admitted or under observation, providing leadership for team members.
• Develop multi-disciplinary discharge plans to optimize length of stay and post-acute care outcomes.
• Engage with patients, families, providers, social services, and utilization management to ensure smooth transitions.
• Review length of stay daily and discharge probability with hospitalists and discharge planners.
• Identify safety hazards and coordinate community referrals as needed.
• Perform additional duties as assigned.
