Job summary

Spotsylvania Regional Medical Center in Fredericksburg, VA is seeking a Social Worker (BSW) to promote patient-centered care by evaluating psychosocial needs and coordinating discharge plans across the care continuum. You will collaborate with a multidisciplinary team to support high-risk patients and ensure timely post-acute interventions.

Qualifications

• Bachelor's Degree in Social Work or related field.
• Basic Cardiac Life Support must be obtained within 30 days of employment start date
• Nonviolent Crisis Intervention must be obtained within 60 days of employment start date

Responsibilities

• Work collaboratively with RN case managers, acute care nurses, physicians, insurance companies, and other staff to identify and address patient needs.
• Identify and address barriers to discharge and recovery including financial, emotional, psychosocial, medical, and spiritual.
• Perform thorough assessments and develop discharge plans for high-risk patients with discharge planning or psychosocial needs.
• Obtain input from the RN case manager to ensure discharge plan aligns with patient care plan and reassess as needed.
• Participate in interdisciplinary care planning and proactive discharge planning.
• Provide education to patients and caregivers regarding community resources and post-acute level of care criteria, benefits, and coverage.
• Coordinate and facilitate meetings between care team and patients/families/caregivers; assist patients in making decisions honoring preferences.
