Job summary

Henrico Doctors' Hospital in Richmond, VA is seeking a Social Worker (BSW) Case Manager PRN to assess psychosocial needs and coordinate discharge planning for identified patient populations in Med/Surg units. You will liaise with the care team and community resources to ensure timely post-acute transitions.

The role requires a Bachelor's in Social Work and up to 1 year of related case management experience; an MSW is preferred.

Qualifications

• Bachelor's Degree in Social Work required
• One plus year related case management experience or BSW internship preferred
• Master of Social Work Degree from an accredited school is preferred

Responsibilities

• Work collaboratively with RN case managers, acute care nurses, physicians, insurance companies, and other staff to proactively identify and address patient needs
• Identify and address barriers to discharge and recovery including financial, emotional, psychosocial, medical, and spiritual aspects
• Perform thorough assessments and develop discharge plans for high-risk patients with discharge planning or psychosocial needs
• Obtain input from the RN case manager to ensure the discharge plan aligns with the patient care plan and modify as needed with the clinical team
• Participate in the interdisciplinary care planning process and perform proactive discharge planning
• Provide education to patients and caregivers regarding community resources and post-acute level of care criteria, benefits, and coverage
• Coordinate and facilitate meetings between the care team and patients/families/caregivers; assist patients and families in making difficult decisions while honoring preferences and values
